AnyMail

AnyMail is a basic form mailer which will allow you to receive the results of any form in email. You can use an unlimited number of form fields, and can specify the email address that each form is sent to, as well as the follow-up page that the user is sent to upon submission. Another feature is the ability to specify one of the form fields as the subject line. You can use as many different forms as you need, all from one script.

MailForm

MailForm will handle any form. The script works with Unix type systems using SendMail, or Windows servers using an SMTP server. If your server does not have SendMail, then there is a workaround to use an SMTP server instead. MailForm will send mail to you from any form you design. The form may be designed any way you like, with any combination of field types. There are a few tags that must be present on your form for the program to send mail to you.
Note:- you need to give an Email address to download the script.

BNBFORM.CGI - The All-In-One Forms Processor

BNBFORM.CGI is an easy to install script that handles an unlimited number of forms. It is configured via the html form: direct data to e-mail and/or file, enforce data entry, autorespond with custom message for each form, redirects based on result of script's validation, sequential counter. A very flexible script.

BS Form

This is a simple form which will take in a users name, email, phone number, message, and message subject and email it to you. The script allows for customizable fields.
